HealthCheck Plus Mini Grant - Application deadline December 31, 2005

If your city participated in HealthCheck Plus this past spring, you could be eligible for a $100 - $600 HealthCheck Plus Mini-Grant—but time is running out!  The application deadline for spring HealthCheck Plus cities is December 31, 2005.

HealthCheck Plus Mini-Grants are used to help you target risk factors identified for your group as a result of participating in HealthCheck.

If you’re not sure if your city qualifies, you can check the required criteria on the AWC Wellness page [www.awcnet.org/wellness]. You’ll want to look for “Coordinator Resources;” select “Forms/Reports” from the resource list. This will then take you to the “HealthCheck Plus Mini-Grant Application” form.
